Applications open <strong>27 August 2010</strong> and the closing date is <strong>24 September 2010</strong>.  All posts have the required educational and Dean&rsquo;s approval.  All interviews will be held on 11 November 2010 in Leeds, West Yorkshire.  Applicants submit one application electronically for all vacancies via ICAMS (electronic recruitment portal), which is accessible via the Yorkshire and the Humber Deanery website.  <br />Training in Paediatric Surgery is consortium based involving a number of regional training schemes. Posts in Scotland will not be included in 2010 recruitment and will be advertised as previously. There are 10 training posts available although currently these are indicative only. There are also a number of LAT&rsquo;s available.<br /><br />Shortlisted trainees will be asked to provide a summary of their portfolio in addition to the full portfolio & an interim educational supervisors report, interviews may involve practical or written stations in addition to face to face interviews.<br /><br />Candidates will be ranked on the basis of performance at interview, application form and Educational Supervisors report.  Successful candidates will be offered training posts according to their declared consortium preference with higher ranked candidates given first consideration.  Offers will be made up to 3 months after the interview round for posts becoming available for start dates up to the end of December 2010.  Candidates will be asked to preference their consortium at application stage and this will be confirmed at interview.<br /><br />Training is managed via training consortia and comprises a six year programme in which the trainee will work in at least two centres within the consortium.  Movement between consortia is unusual and only possible via application to the Lead Deanery of the consortium.  Where possible consideration will be made for the Trainee&rsquo;s circumstances when allocating postings within the consortium, but training and local service needs will dictate individual rotations at the discretion of the local Programme Director.<br /><br />Higher specialty training in paediatric urology is offered by some centres in the form of fellowships which are outwith the general training programme in that centre. The question you ask yourself is why you wish to work overseas, for how long, in what capacity and how will it benefit your training. The IAC (international affairs committee) have set out some guidelines to assist you, however you need to tailor your plans to suit your requirements. Whatever kind of post you do overseas, you should try to keep in touch with your Programme Director regularly since there may be changes happening in the UK which you need to know about. This is especially important if you are considering going away before you have a run through NTN e.g. after &ldquo;Core training&rdquo; before specialisation. Considering the timing and preparation for exit examinations and any subspecialty interviews is also important. Careful discussion with your Programme Director at an early stage of planning is the wisest course of action. It will address the challenging problem of weaning patients from intravenous nutrition on to enteral feeding and maximising the use of the gut. <br />It will deal with the practical problems encountered in the day to day care of patients with intestinal failure. <br />The areas covered will include the indications for intravenous nutrition, formulations commonly used, recent advances in intravenous nutritional constituents (including use of glutamine, types of lipid available), prevention of septicaemia, management of blocked lines, surgical procedures for improving intestinal function (bowel lengthening, indications for and types of stoma) and gastrointestinal investigations useful in evaluating intestinal failure. There will also be lectures on the importance of enteral nutrition in intravenously fed children, devices for delivering enteral nutrition and their indications, calorie requirements and monitoring of nutrient status. The importance of recognising motility disorders will also be addressed.

It is increasingly unlikely that the government will allow significant derogation and personal opt outs are not a viable solution either to the training or workforce issues.<br />Trainee numbers have expanded in recent years, the impact of this is uncertain. There are concerns that it will lead to over training and the possibility of post-CCT unemployment. The high proportion of females trainees in paediatric surgery make the situation harder to predict, as does the current uncertainty over the future of GPS provision. There is no evidence that post-CCT unemployment is occurring in our specialty but the numbers of applicants for each consultant job has apparently increased in the recent past and this issue remains a cause for concern.<br />In Scotland, training numbers have been reduced across the board and the rest of the UK is likely to follow. In the long term it is probable that many non-training middle-grades will be required to cover on-call rotas while training appropriate numbers. In the meantime, the JCST&rsquo;s recommendation that trainees at ST3 and above do not work night shifts will remain aspirational for paediatric surgery.<br /><strong>Training</strong><br />PMETB have emphasised the importance of externality on ARCP/RITA panels. If you are concerned that an ARCP/RITA panel has no external panel member then you should raise this with you program director or, if you feel unable to do this, the SAC liaison member for your consortium.<br />Core surgical training has been extended to three years but some trainers feel they can deliver the curriculum and their trainees demonstrate the necessary competencies in two years. How this will be resolved is currently unclear.<br />Only those who have completed an eight-year training program in posts prospectively approved by PMETB can be awarded a CCT. Those who completed some alternative training can only be awarded a CESR as their route to the specialist register. This will mainly affect those who qualified overseas but may affect UK trainees as well. If any one is concerned about this they should contact their deanery or the SAC but the suffix on their training number shows what end point of training they should expect: Numbers ending XXX/C denote a trainee on route to a CCT, XXX/E denotes a CESR.<br /><strong>Recruitment</strong><br />Paediatric surgery will change to a national selection process for ST3 posts in 2010, it had been planned for 2009 but the uncertainty as to the effect of run-through appointments at ST1 level in 2007 made it impossible to know how many posts would be available this Summer. Scotland has already, in effect, already run national selection and may join the rest of the UK in 2011. It is unknown how Northern Ireland will recruit.<br />The details are unclear but there will be a single advert and applications will be handled by one deanery. Applicants will be able to specify deaneries of interest to them. Interviews may be national or local but will in a standardised format.  It is anticipated that the best candidates will be offered first choice of jobs.  It is not yet known if LAT and ACL/F posts will be included and there may be one or more rounds of recruitment each year.<br />The advantages of a national process are several-fold: It is cheaper and more efficient for the deaneries, trainees will not have to complete many applications and will have offers of posts released in a coordinated fashion, avoiding the &ldquo;stick or twist&rdquo; dilemmas of the past. Appointable but unsuccessful candidates may be kept on a register and offered unfilled posts.<br />A personal specification has been drafted and will be publicised well in advance of advertising jobs in order to allow trainees time to prepare their CVs. It is very similar to the personal specification used to appoint to SpR posts on the Calman system. A significant change to previous personal specifications is the removal of a maximum amount of time spent in paediatric surgery prior to appointment at ST3. This is because of legal difficulties in stipulating maximum experiences and is awaiting final clarification.<br />The trainee representative was supportive of the principle of national selection in general and the proposals as they stand. The unit comprised 4 teams, each with 2 or 3 Consultant surgeons, a&nbsp;Registrar and Resident on each team. There is more clinical and operative exposure during the standard working week than during a comparable week on a surgical unit in the UK, where one registrar would often only be working for one Consultant surgeon. The Consultants take the surgical training very seriously with teaching sessions at 7.30am on 3 mornings during the week. In addition to the clinical experience, it is an excellent preparation for the FRCS(Paed).<br />Melbourne has a huge experience of treating oesophageal atresia including oesophageal replacement - experience that it would only be possible to gain in some of the units in the UK. The proportion of cases performed using minimal access (either laparoscopic or thoracoscopic) techniques is still relatively high compared to many UK centres. Exposure to trauma is excellent, with the on call surgical registrar usually taking the responsibility of leading the assessment of the trauma patient, and there is also training in burn management. The urology exposure is also very good.
